  • Pilonidal means a'nest of hairs.
  • A sinus tract is a narrow tunnel (a small abnormal channel) in your body. A sinus tract typically goes between a focus of infection in deeper tissues to your skin surface. This means that the tract may discharge pus from time to time on to your skin.

A pilonidal sinus is a sinus tract which commonly contains hairs. It occurs under your skin between your buttocks (the natal cleft) a short distance above your back passage (anus). The sinus tract goes in a vertical direction between your buttocks. Rarely, a pilonidal sinus occurs in other sites of your body.

What Causes a Pilonidal Cyst?

Most doctors think that ingrow hairs are the reason for many of them. Pilonidal means "nest of hair", and doctors sometimes find hair follicles inside the cyst.

Another theory is that pilonidal cysts appear after a trauma to that region of your body.

During World War II, more than 80,000 soldiers got pilonidal cysts that put them in the hospital. People thought they were because of irritation from riding in bumpy Jeeps. For a while, the condition was called "Jeep disease."

You might be more likely to get one if you were born with a small dimple in the skin between your buttocks. This dimple can tend to get infected, though doctors aren't exactly sure why.

What are the signs and symptoms of a pilonidal cyst?

Certain individuals with a pilonidal cyst may not have any symptoms at all, and the only finding may be a dimple or an opening in the skin (sinus tract) near the cleft of the buttocks. However, if the pilonidal cyst becomes infected (pilonidal abscess), the following signs and symptoms may develop:

  • Pain over the lower spine
  • Redness of the skin
  • Warmth of the skin
  • Localized swelling over the lower spine
  • Drainage of pus from an opening in the skin (sinus tract) over the lower spine
  • Fever (uncommon)

Though much less common, pilonidal cysts can also develop in other areas of the body, such as the hands.
